Remove iPhone Value Pack

I would like to know how to remove the Add-On iPhone Value Pack. Because this Add-on includes 

-Unlimited text, picture and video messages from Canada to US

-Premium Calling Rate

-Visual Voicemail Plus

...

So when I want to remove this service calling the *234, the options are to remove every service but not the complete iPhone service.

Someone knows the combination of numbers when you call *234 to desactivate this service?
