I just upgraded my phone with FIDO and my new phone came with a card (the bigger credit card size) that says Multi Sim card. It has a place for the sim card but, it's empty - the sim card is already punched out. So I'm wondering if it is missing or since it's an upgrade am I supposed to just transfer my old sim card?

If they provided the Multi-SIM card, it should be present. It's possible the actual SIM was dislodged during shipping. Have you checked to see if the SIM might found among the packing materials?
That said, if your new phone uses the same sized SIM, you can simply transfer your SIM to your new phone.
If your new phone uses a different sized SIM and you are unable to find the new SIM, you might consider requesting a new SIM from customer service. Alternatively, you might consider sending @FidoSolutions a PM. Once they verify some information, they'll also be able to access your account.
