how to make a service request for my device protection plan? what are store locations that does the services because fido told me that its a different store.

Welcome to the community!
All repairs under the device protection must be done through one of BrightStar's authorised partners. You can start a service request via My Account --> Profile & Settings --> Usage & Services --> Mobile --> Quick Actions (near bottom of page) --> Repair or trade in device.
Once you complete your service request, you will be provided with Brightstar's repair partners in your area (see here).
