How I return modem. Store not accepting

I'm a participant level 1

I cancelled my home internet service and have not received any confirmation of the cancellation by email.  And I went to return my modem in store (which on your website specified "Get the Right Hardware (exchange/swap equipment)".  But they advised they don't accept the equipment.   So I need to send it back via mail.  Can you please send me the box and mailing label to return it?  Thanks!
