Vivaz Pro error code when setting new PIN?

I was trying to turn on the PIN, but it kept on sending me an error code.

It locked me out with the PUK, and once I entered that, it told me to make a new PIN number. After I tried verifying the new PIN, it always says 'Code Error'.

It isn't that I haven't entered the right PIN twice, and the phone won't seem to let me in without setting a new PIN. Help?

Try entering 1234 as the new PIN. If you enter the PIN and PUK incorrectly too many times you may brick the sim card, once that happens the only solution is to get it replaced.
