Samsung Galaxy Q (T-589R), camera autofocus and the mysterious lock icon

The manual for this phone claims it has autofocus ("3.0 Megapixel camera and video with autofocus") but this doesn't seem to work at all. Also, the camera will sometimes display a lock icon (looks like a combination lock, appears toward the top right of the screen, to the left of the remaining shot count. I don't know what causes this, but when it is on, the phone is locked. It times out eventually or I can power down the phone or force the camera app to exit.

 

I don't see it described in the manual anywhere.

 

Is this phone screwed up, or is this new user screwed up?

Well, I figure out where the lock comes from (but not why...) - but the focus remains a mystery.
