Cancelling my mobile contract

I am cancelling my mobile contract for a mobile phone I am paying that has not working since April 2016. It's frusrating to lost of money for a service not used.  

 

My contract end date is not in my account profile but I believe its December 2016.  How do I cancel my contract?
